Process Overview - Easy As 1,2,3

There are three steps to registering your student:

  1. Collect required documents

  2. Submit digital copies of required documents

  3. Fill Out Special Programs Survey Online Form

Step 1 - Collect Required Documents

Download and complete all necessary documents before starting Step 2.

Required Uploaded Documents

UTK-4 Enrollment Form - Download this form.

You will upload your completed form during Step 2.

Home Language Survey Form - Download this form. You will upload your completed form during Step 2

Birth Certificate or Passport - You will upload this document during Step 2.

Your child must be five years or older on or before September 1 to be eligible for Kindergarten. If they turn 4 before September 1, they are eligible for Universal Transitional Kindergarten.

Immunization Record - You will upload this document during Step 2.

Kindergarten/Transitional K immunization requirements must be completed before the student can be placed in a classroom - CA Codes section 120325-120380 - Click for required immunizations

Proof Of Residency - You will upload this document during Step 2.

Upload your most recent utility bill, cable bill, tax bill, mortgage document or military housing orders. Upload the first page to show name & address.


The Following 2 forms are required for Kindergarten within 30 days of the start of school. YOU DO NOT HAVE TO WAIT TO SUBMIT your enrollment.

Medical Examination Form - Download this form. (DUE by Sept. 13)

Please DO NOT submit the other required enrollment forms to this email. You will be downloading these forms in STEP 2.

Downloadable Documents

A PDF of each required form must be downloaded, completed, and saved on your computer. You will then upload the completed copy of this form in Step 2. You can complete electronic forms without printing by using the free Adobe Reader DC. (Click here for Instructions on how to download, install and use Adobe Reader.) You can take a photo of the completed form and submit it if you are unable to use Adobe Reader.

Uploading documents from Google Drive or other online storage will not work. All documents to be uploaded MUST be stored on your computer, tablet or smartphone. You will receive an email verification once your files have been uploaded.
